I built this decision tree because I kept seeing the same gap across my own field: risk assessment treated like a light switch, someone is safe or they’re not, and the response treated the same way, proceed as usual or hospitalize. That’s not how risk actually works. The real clinical decisions live in the middle, in questions about a client’s support and environment, how quickly their status is shifting, and the difference between someone being willing to safety plan and actually being able to.

This is the framework I use in my own practice to work through that middle ground, step by step, from initial concern to appropriate intervention.
